Notification of Data Security Incident

June 18, 2025 – On or around September 16, 2024, Galleria Woods, discovered suspicious activity related to an employee email account and immediately began investigating with the assistance of third-party computer forensic specialists. The investigation determined that certain data was accessed by an unauthorized party from September 9, 2024, through September 13, 2024. While Galleria Woods is currently finalizing their review of the information potentially impacted by this event; Galleria Woods has confirmed that a limited amount of protected health information may have been impacted in connection with this event.  Although Galleria Woods has no evidence of actual misuse of information as a result of this event, Galleria Woods is notifying potentially impacted individuals out of an abundance of caution.

The types of information impacted by this event might have included individuals’ first and last names in combination with their address, date of birth, Social Security number, driver’s license or state identification card number, USCIS or alien registration number, financial account number, financial account access information, payment card number, payment card access information, account username and password information, medical record number, patient account number, health insurance account number, health insurance group number, mental or physical diagnosis information, medical treatment or procedure information, clinical information, prescription information, provider location, and/or provider name. Galleria Woods has established a dedicated assistance line to answer questions about the event and to address related concerns. The number for the dedicated assistance line is (855) 201-9134 and representatives can be reached Monday through Friday, from 8:00 AM to 8:00 PM Central (excluding major U.S. holidays). In response to this event, Galleria Woods has implemented additional security measures within its systems and facilities and is reviewing its current policies and procedures related to data security.  While Galleria Woods has no evidence of actual misuse of information as a result of this event, it is always prudent to review health care statements for accuracy and report any services or charges that were not incurred to the provider or insurance carrier. As a best practice, individuals are encouraged to remain vigilant against incidents of identity theft by reviewing account statements, credit reports, and explanations of benefits forms for suspicious activity and to detect errors.
